Intense “prehab” — ACL strengthening programs before surgery — improves knee strength and increases your chances of a successful return to sport.

To ensure your ACL reconstruction (ACLR) has the muscular support it needs, thorough pre- and post-surgery rehabilitation is essential.

During prehab, we also take baseline measures using VALD Force Plates. This data guides our decisions for a safe and efficient return to sport, helping you recover as quickly and safely as possible.

ACL Surgery or No Surgery?

If you’ve torn your ACL, you have options — and physiotherapist-guided rehabilitation is essential, whether or not you choose surgery.

Research shows no significant difference between immediate ACL reconstruction followed by rehab and starting with rehab before surgery (Frobell 2013), highlighting that rehab alone can be a primary treatment option.

The decision to have surgery or manage non-surgically depends on factors such as:

  • Age and timing of the injury

  • Knee instability at rest or during movement

  • Meniscus or other associated knee injuries (e.g., MCL sprain)

  • Desire to return to sports involving jumping and pivoting (rugby, AFL, soccer, skiing, volleyball, netball, etc.)

  • Occupational demands (e.g., active jobs vs desk work)

Returning to Sport After an ACL Injury

Recovery after ACL reconstruction is individual and depends on your goals. Before returning to sport, you need to safely and confidently pass physical tests that demonstrate readiness.

Confidence in your knee, along with completing sport-specific drills, is essential (Thomee 2011, Arden 2011). Your physiotherapist will guide you through a series of tests, including hopping exercises and strength assessments using specialised equipment, to ensure a safe and effective return to your sport.

Risk of Re-Injury After ACL Rehab

There is a small chance of re-injuring your ACL when returning to sport, and this risk applies to both your injured and non-injured leg. The good news: following your ACL rehab plan until your strength, stability, and mobility are fully restored can significantly reduce this risk (Manske 2012). This process usually takes 12 to 18 months.

ACL Testing

Research shows that insufficient rehabilitation and lack of objective knee testing, combined with returning to sport too early, can increase the risk of a second ACL injury and affect performance.

At Narrow Gate Physiotherapy, we use our specialised ACL Return to Sport Testing program, providing objective measurements similar to those used by elite sports clubs.

Do you treat men and women?
While we have a huge heart to look after men who struggle to be understood in the health industry generally, but also in the realm of men’s health physiotherapy (pelvic floor, incontinence, pelvic pain, prostate and sexual dysfunction).. Our musculoskeletal, sports and aged care services are available and effective to all populations including women and children. In fact, around 50% of our clients are split between men and women.

We offer:

  • men’s health physiotherapy,
  • musculoskeletal physiotherapy (pain and dysfunction associated with everyday life including work, home activities, hobbies and posture etc.),
  • sports physiotherapy,
  • work cover/ctp (injuries happening at work and covered by injury insurance),

Postural dysfunction is common in sedentary populations like desk workers and people engaged in repetitive work that requires prolonged postures. We look holistically at the neck, back and shoulder joint and muscular imbalances to quickly solve this issue for good.

Sports injuries vary from gym-related, soccer, basketball, track and other high impact activities. We work closely with your goals and coach to take an ‘active recovery’ where you are still maintaining strength, flexibility and cardiorespiratory condition whilst your primary injury is healing. No, although a referral is always a good way for your current medical practitioner to give us a head start on understanding your medical history.

Special referrals from your GP called EPCs (Enhanced Program of Care) can be sent to us for chronic conditions (> 3 months since incidence) that can attract a government rebate of up to 5 physiotherapy sessions per year.

We accept all private health insurances on our HiCAPS machine that will instantly process your rebate claim on the spot in our clinic.

Invoices are provided on demand to you, especially for overseas insurances for you to process yourself online.

Yes! We offer telehealth for all our services. Just let us know that you would like this over the phone or in the notes of your online booking and we will reach out to you with instructions on how to best set up for your appointment.

Generally, we use a native telehealth function in our practice management system called Cliniko where you will receive a link to a video conference via email. Please have a stable internet connection, clear space around you, bluetooth earphones and a good quality camera (contemporary phone and laptop cameras are great) for a great experience.
